somew1nd 5d92939d91 refactor: unify agent history and harden runtime diagnostics
Preserve user guidance through history compression, stabilize Flash index targets, and retire withdrawn checks without applying stale verdicts.

Improve runtime diagnostics, tool responses, and history rendering. Simplify documentation and comments.

Validation: 2076 tests passed in the full suite; 381 affected tests and 26 frontend tests passed after cleanup. Ruff and the quality ratchet passed.

"""History compression into segment chunks, eras, and recall-only periods.
Each chunk keeps three bands: synopsis and effects, interval summaries, and a
per-step action ledger. Capsule generation reads the operator's turn transcripts
alongside the recorded step facts. Milestone changes, segment size, and token
limits determine chunk boundaries.
Original messages remain in context until their capsule is ready. Failed capsule
jobs retain their source text for retry. At the hard token limit, ready chunks
swap as ordinary L2 blocks first; only when that cannot bring the context back
under the line does everything closed force-swap (pending chunks included) and
the whole frozen region fold into a recall-only era — L3: the period paragraph
plus the per-step minimal index (step number, session offset, action phrase).
The fold is monotonic: later L2 swaps append new chunk blocks after the folded
eras and never restore the folded chunks. Older chunks also fold into eras and
then recall-only periods by count overflow; full step records remain available
through the history tools.
Checkpoint annotations persist independently of capsule generation. Frozen
context is rebuilt only when a compression event replaces or folds turns.

Swaps consume the transcript's oldest unchunked turns, so only a
contiguous READY prefix may swap — an older pending segment keeps its
(and every younger segment's) original text live.
Start gate: closing a segment (milestone/size triggers) only *prepares*
its capsule in the background; the original text is replaced only once
the operator's measured context has reached ``budget * start_ratio``.
Below that the ready chunks are held in the awaiting queue, so at low
occupancy the model keeps reading the full transcript while capsules
are already on hand for when pressure arrives. An unknown base (no
measured operator call yet, or a provider without usage metadata)
cannot gate and falls through to swap-on-ready.
Hard threshold ladder (L2 first, L3 last): at ``budget * hard_ratio``
the ready prefix swaps as ordinary L2 chunk blocks when the estimated
context after that swap drops back under the hard line (rung 1). Only
when no ready prefix exists, or the L2 swap would still leave the
context at or above the line, does everything closed force-swap —
pending chunks included — and the whole frozen region fold into a
recall-only era (rung 2: L3 = period paragraph + per-step minimal
index). With nothing awaiting at all, the last resort is folding the
existing frozen region the same way without consuming turns (rung 3).
The fold is a monotonic state change: later L2 swaps append new chunk
blocks after the folded eras and never restore the folded chunks;
repeated hard renders with nothing left to fold leave the frozen
blocks byte-identical.
``base_tokens`` is the prompt size before the swap, used to estimate
the remaining context on the last swapped chunk's trace.

A turn without a stamp (no recorded step — a reply without a tool
call, a helper-only turn — or a step not stamped yet) joins the
running segment. Leading unstamped turns have no running segment to
join: they are held and become the head of the first stamped segment
instead of forming a stampless segment of their own, which would
close as a chunk with no step records (``_create_chunk`` returns
``None`` for it) and sit at the head of the awaiting queue.
