新增阶段1预审决定标准，涵盖未知项分类（RISK、INPUT_REQUIRED、UNCERTAINTY、DEPENDENCY、ASSUMPTION）、可行性风险判断原则、专业角色触发建议及输出与阶段边界，明确历史等价覆盖范围、输入依赖和重评条件，避免以未知项代替风险或弱化阶段2/3完整评估。
